Pre-Registration

Thank you for choosing to have your surgery or procedure with us at the Marin Specialty Surgery Center.  It is very important that we contact you prior to your procedure.  A business office representative will contact you to verify the information we have received from your physician’s scheduling staff.  We will confirm any insurance coverage (if applicable) and any fees that you may be responsible for before your date of service.  One of our clinicians from our pre-op team will also call you to review and prepare you for your day of service.  If your procedure requires anesthesia, an anesthesiologist will also contact you the day or evening before your surgery.   You will be asked questions regarding any prior surgeries and your past medical history.  Your physician’s office staff may also have given you an admission packet that includes a welcome letter, patient rights and responsibilities, our Privacy Practices, directions to the surgery center, and a pre admission questionnaire.  Please complete all paperwork and get it to us.  Directions for how to do so are in the packet.

The federal government requires that we inform you of three important issues:

1.  ADVANCED DIRECTIVE INFORMATION (LIVING WILL / HEALTHCARE PROXY)

The Marin Specialty Surgery Center‘s policy on Advanced Directives is as follows:  

  • You have the right to make choices regarding life-sustaining treatment, including resuscitative measures.
  • As your procedure here is elective, MSSC cannot withhold treatment if an unexpected complication were to arise. 
  • If there is a need to transfer you to a hospital for additional care measures beyond what we can provide, your Advance Directive will be in your record and honored at the hospital or with your next caregiver upon your arrival.  We are required to inquire about your legal directives regarding your medical care. However, you are not required to present an Advance Directive document before you have surgery in an ambulatory care surgery center.
  • Please do not bring a blank Advanced Directive form to the Center - it must be completed and witnessed prior to your surgery if you wish to include it in your patient record.

2.  PATIENTS RIGHTS AND RESPONSIBILITIES - English

You have a RIGHT to: chose your provider, considerate and respectful care, privacy, confidentiality, responsive service, access to your records, understanding your fees, interpreter services, written notice of your protective information and understanding your care. 

You have RESPONSIBILITIES to:  answer question fully, make sure you understand, be open, follow the prescribed plan, report changes, know your providers, respect fellow patients, the staff and the facility; pay your fees and maximize healthy habits.

 

3. PHYSICIAN OWNERSHIP

Your surgeon may be a principal, or physican investor, in the Marin specialty Surgery Center, which is a proud partner of Surgical Care Affiliates.  We want to offer our patients a compassionate, safe, cost-effective, alternative to hospital-based care.  Over forty surgeons are privileged on our medical staff; twenty-seven of whom are limited physician partners.  While MSSC has been recommended, the choice of where you would like to have your surgery is solely yours.  If you would like us or your doctor to provide you with alternative venues for your surgery, please speak up and we will inform you about other out-patient surgery choices.  

You must be pre-registered prior to the day of surgery, federal regulations require that we speak with each patient or guardian or the surgery must be cancelled.
